When registering your student for the Rosedale Center, you will be asked to complete and submit the following:

- Consent for Release of Confidential Information
- Consent for Release of Student Records (Rule 5230 - Form A
- Annual Consent for Administration of Discretionary Medications and Health Contact       
  Information 
- Student Laboratory Safety Contract
- Student Device Loan and Use Agreement
- Parent and Student Sign-Off for Guidelines and Procedures
